	Department Forms
	2009 Fire Tag [ 1,288 KB ] The 2009 fire tag form can be printed and filled out by hand and then mailed to the department at the mailing address provided on the form. If you are a first time tag purchaser a one time fee of $5.00 will also be needed for a membership fee. The total will then be $65.00. Any alteration of the original wording on the fire tag form will subject to void the form and contract. Payment and forms can also be dropped off at the fire station when personnel are there.
	Applicaton [ 83 KB ] The Wolf Creek Fire Department is always accepting applications for the position of Firefighter. If you would like to help your community click on the application link and print out the application that appears. We are especially looking for volunteers that can respond during the day but are accepting applications for those that can respond at all times of the day. Experience is not required but is a positive. The Wolf Creek Fire Department has an entry program that will train new Firefighters over their first year of service. (Rookie Year)
